Balanced agonism of GIP receptors (incretin amplification, adipocyte lipid metabolism, neutral-to-positive insulin sensitivity effects) and GLP-1 receptors (glucose-dependent insulin secretion, glucagon suppression, satiety, delayed gastric emptying). Albumin-binding C20 fatty acid chain enables once-weekly dosing.
Standard titration: 2.5mg weekly (weeks 1–4) → 5mg (weeks 5–8) → 7.5mg → 10mg → 12.5mg → 15mg weekly maintenance, escalating every 4 weeks based on tolerability. Subcutaneous (abdomen, thigh, upper arm). Research protocols typically 24–72 weeks.
